For your initial consultation, please remember to bring:
- A current referral letter from your GP or other specialist doctors
- Medicare Card or DVA Card
- Private Health Insurance information (if applicable)
- Hard copies or electronic versions (ie CD) of scans (x-rays, CTs, MRIs, PET scans or Ultrasounds)
- List of current medications
- If Workcover or TAC related, please bring approval letter and claim number
